After review of the three potential continuous glucose monitoring systems -- Freestyle Navigator investational device, Dexcom Pediatric Sensor investatgional device, and the Medtronic 722 system which is currently FDA approved for use in patients 18 and older which would be coupled with the Guardian Clinical __________ device -- pt and her family express their desire at this time to use the __________ Guardian Clinical and 722 system.

As such, pt and her family were instructed on the use of the Guardian Clinical ___________ system.

All three blanks s/l BLINDED. Fairly clear dictation but keep in mind dictator is a NP -- and thus, she may have lost her mind. Gah!
