help with anal fistula word sm

This doc keeps saying something that i cannot make out, as he is a bad mumbler. The patient has an anal fistula and Crohn's. He keeps using a term that sounds like "sea tongs" and once he said "super sea tongs". He just said "the s/l sea tongs are now quite loose, the tissue is softened up significantly and he now wishes to have the s/l sea tongs adjusted since they are hanging quite loosely."

Please help!
