The ulcer was debrided aggressively with a curette and the above dressing applied. We will see him back in two weeks. A Wound V.A.C. may be our best option to enable the wound to shrink. An S/L "apple graft" is another possibility. He is certainly not a candidate for either revascularization or extensive type of coverage procedure.

This is a heel ulcer.
