Just doing an H&P here...

He carries a diagnosis of severe sinus infection, was hospitalized in January 2004 at xxx Hospital in xxx, California. He had a probable s/l "mirr-koller" infection. He was in the hospital for approximately one month and released.

Any ideas? THANKS!!!
