Help with this phrase:

Under heart examination:
Regular rate and rhythm. Grade 1 murmur, PMI 2 RSV (s/l) hear welch at the precordium.
I suspect he is saying "heard well" at the precordium, but what is the PMI 2 RSV referring to and how do I punctuate it? Thanks!

Note the size of the PMI (PMI is normally 2-3 cmin diameter). (he could be referring to size of the PMI)
RSV is respiratory syncytial virus.....doesn't fi
as for the RSV...are we sure that is what he is saying? (nm)
