Does this look okay?

Pt was taken to the operating room where under general anesthesia a cystoscopy and placement of ureteral stents was performed. After which an exploratory laparotomy was performed demonstrating the uterus to be diffusely adherent to the abdominal wall and extensive adhesions obliterating the anterior cul-de-sac. TIA

or maybe......was performed. After this, an exploratory laparotomy was performed which demonstrated the uterus to be diffusely adherent to the abdominal wall, with extensive adhesions....

Do you think "was" performed should be "were" performed? I'm going back and forth on that.
